The National Drug Codes (NDC) 0023-3919-50 and 0023-9232-01, specific for the Botox Cosmetic product, are not a covered benefit for Blue Cross Blue Shield of North Dakota (BCBSND) Commercial or Medicaid Expansion members.
Claims submitted with the Botox Cosmetic NDC numbers listed above will be denied.
